To confirm that you agree with the fees for services, please submit the form below. This form should be completed by, or with, the potential client.

The UNC Department of Psychology Community Clinic is a community-based training facility for the Ph.D. program in clinical psychology. We are dedicated to providing the highest quality training for our graduate students as they learn evidence based psychological treatments. Fees from services provided are used by the Clinic to further this endeavor.

Thank you! We hope to respond to your inquiry within a couple of business days. At the start of the academic semester, August/September and January/February, we receive a large amount of inquiries and may be slow to respond.
